Reporter Xu Lijuan / Report from Kaohsiung

A 20-year-old young woman in Kaohsiung had a serious car accident in mid-February when she crashed into a roadside truck. When the woman was transferred to the Affiliated Hospital of Kaohsiung Medical University, in addition to serious facial injuries, the doctor also suspected intra-abdominal bleeding. There were no traces of bruises from the impact of external forces, but a computerized tomography scan revealed that part of the liver had been shattered, and the pancreas had also been broken in two. Emergency surgery was required to save this young life.

Wu Baixuan, a senior general and digestive surgeon who performed emergency surgery on the patient, pointed out that this is a relatively rare case. Except for the severe impact on the abdomen but no bruises, the liver or spleen is usually ruptured.

Since the pancreas is located in the very center of the abdomen and is a relatively long, flat and soft structural tissue, it is rare for a direct impact to cause it to break into two halves. It is speculated that it is due to the shear force of "acceleration-deceleration" The torsional force after deceleration destroyed the organs abruptly, and it could be said that the internal organs were torn apart.

Wu Baixuan said that during the operation, the patient's blood pressure kept dropping, and it was a very big challenge for doctors to perform liver and pancreas operations at the same time.

Hepatopancreas rupture found in computerized tomography

Because a part of the liver was necrotic, 1/4 of the liver was removed, and the pancreas was irregularly disconnected and could not be repaired, so the disconnected part was urgently removed, and 1/2 of the part connected to the blood vessels was retained. Fortunately, the other gastrointestinal tract The patient was completely intact. The patient was hospitalized for nearly a month after the operation, and was not discharged until mid-March. He is recovering well. Currently, his digestion, liver and insulin functions are normal, but his memory of the car accident has been blurred, and he has no idea where he was. Walk before the gate of hell.

A broken pancreas is actually very troublesome. Wu Baixuan said that the function of the pancreas is to secrete pancreatic juice, which helps the gastrointestinal tract to digest and has the function of enzymes. If too much is removed, the function of digesting fat will be reduced, so some patients will have fatty stools; moreover , The pancreas also secretes insulin. After resection, there is a chance to become diabetic in the future. At present, the blood sugar level and function of this young patient are normal, but continuous observation and follow-up is required.

Wu Boxuan reminded that in case of a car accident or accidental impact on the abdomen, special care should be taken within a week. If there is no discomfort at the moment, but abdominal pain intensifies, weakness or loss of appetite occurs over time, it is recommended to return to the clinic for further examination.
