Mr. Lin, 45 years old, has often felt pain and discomfort in the right upper abdomen for the past six months. He started to take stomach medicine to relieve it, but recently the pain has gradually intensified, even to the back, and he has insomnia at night.

One night, I suddenly had two or three consecutive stools, all of which were black and mushy, accompanied by dizziness and discomfort. I rushed to the emergency room. After gastroscopy, I found a duodenal ulcer and bleeding, as well as Helicobacter pylori. Fortunately, after treatment with antibiotics and other drugs, The symptoms disappeared.

Cause chronic gastritis and even gastric cancer

Shao Changhan, Physician of Gastroenterology and Hepatobiliary Department of Dali Ren'ai Hospital, said that Helicobacter pylori (referred to as pylori) is a bacterium that resides in the human gastric mucosa. About half of the world's population carries the bacteria. The bacteria can exist in feces, gastric juice, vomit or contaminated Water quality, it is speculated that the infection is related to the environment and hygiene habits.

Generally, infection occurs mostly in children, and adults are less susceptible to infection. Chronic pylori infection can cause chronic gastritis, gastric and duodenal ulcers, and even gastric lymphoma and gastric cancer.

Shao Changhan pointed out that about 80% of patients with pylori infection are asymptomatic carriers, 10-15% have chronic atrophic gastritis or peptic ulcer, and only 1% develop into malignant gastric cancer or lymphoma. The World Health Organization (WHO) has In 1994 it was defined as the first class of carcinogens.

Statistics and found that after pylori treatment from 2004 to 2018, the prevalence of pylori infection dropped from nearly 70% to 10%, the incidence of gastric cancer decreased by 53%, and the mortality rate of gastric cancer decreased by 25%, showing that the eradication of pylori infection can Effectively reduce the occurrence of gastric cancer.

Shao Changhan said that gastritis caused by chronic pylori may develop into irreversible gastric mucosal atrophy or even gastric mucosal lesions, which will greatly increase the risk of cancer.

Therefore, to screen and treat pylori bacteria, it is recommended to effectively suppress the canceration before the above irreversible lesions occur. The current treatment method is to use three-in-one antibiotic sequential therapy for 7-14 days, and the eradication rate of pylori bacteria can reach 80-90%. .

Shao Changhan reminded that if there is long-term discomfort in the upper abdomen, it should not be underestimated, and should be discussed with the doctor as soon as possible for further examination.

It is also recommended that patients with confirmed peptic ulcer, dyspepsia symptoms under the age of 60, patients with unexplained iron deficiency anemia, patients with gastric mucosa-associated lymphoma, and patients who have been treated for early gastric cancer need to take anti-platelet/non-steroidal anti-inflammatory drugs for a long time Those who can accept pylori screening.
